Must bring copies of your:

• Copy of Current electric bill (must show your name, full address, account number and amount due)
• Copy of Valid Photo ID (driver’s license, permanent resident card, passport, etc.)*
• Copy of Social Security cards for all household members
• Copy of Proof of income for the last 30 days for all occupants 18+**

*If someone is representing an applicant, they must provide a copy of their state ID and a signed letter from the applicant, giving permission to sign on their behalf
** ANY household member with No Income must complete a zero-income form during the appointment

4: $66,441
5: $77,071
6: $87,702
7: $89,695
8: $91,688

In households with 9+ persons, add $1,906.00 for each additional household member.

Please note:

• Failure to provide ALL required documentation will delay your assistance.
• Service bill can not be over $1000 Credit.
• Available until funds are exhausted.
