To register as a table captain, a sponsor or to RSVP your individual seat, please make your selection below.

You will see there are fields for you to fill in your personal name and information. You will receive an email confirmation after submitting this form. In your email, you will find a form that will allow you to register your guests at a later date. We kindly ask that all guest registrations be submitted before May 5.
